Mechanism of Action

PMI-5011 is a standardized ethanolic extract of Artemisia dracunculus (Russian tarragon) characterized by a polyphenol-rich fraction containing 2',4'-dihydroxy-4-methoxydihydrochalcone (DMC-2), 2',4'-dihydroxy-4-methoxychalcone (DMC-1), davidigenin, and other chalcones alongside caffeic acid derivatives. These compounds collectively enhance insulin signaling by increasing tyrosine phosphorylation of insulin receptor substrate-1 (IRS-1), elevating phosphorylation of Akt (protein kinase B), and stimulating translocation of GLUT4 from intracellular storage vesicles to the plasma membrane in skeletal muscle — mimicking and potentiating the actions of insulin at post-receptor signaling steps. Additionally, PMI-5011 inhibits protein tyrosine phosphatase 1B (PTP1B), a negative regulator of insulin receptor signaling, amplifying insulin signal transduction in insulin-resistant tissue. Hepatic glucose output is reduced through PEPCK and G6Pase downregulation.

Well-characterized animal studies by Ribnicky, Poulev, Raskin and colleagues (Phytomedicine, 2006; J Nutr Biochem, 2014) demonstrated that PMI-5011 at 500–1000 mg/kg reversed insulin resistance and reduced hyperglycemia in high-fat-diet ob/ob mice and Zucker fatty rats, with IRS-1 and Akt phosphorylation confirmed by Western blot in skeletal muscle. Two human pilot RCTs (Cefalu et al.; n=25 and n=40, respectively) found PMI-5011 (500 mg/day and 1000 mg/day) significantly improved insulin sensitivity by 18–28% (measured by hyperinsulinemic-euglycemic clamp) and reduced postprandial glucose excursions versus placebo over 4–8 weeks in metabolic syndrome patients, with no significant adverse effects. These trials are small and require replication in larger multicenter RCTs.
