Clinical trials of daidzein-rich isoflavone extracts for menopausal symptoms show modest benefits for hot flash frequency in some but not all studies. The equol hypothesis—that only individuals whose gut microbiota can convert daidzein to the more potent metabolite S-equol derive full benefit—has been supported by subgroup analyses. For alcohol-related effects, preclinical studies in alcohol-preferring rats show daidzin (the glycoside form) reduces voluntary ethanol intake by approximately 50%, mediated partly through ALDH2 inhibition.
